The following documents should be included with your Coach Application.  A list of BMLA Coach requirements can be found on page 2 of the Application Form:

þ Certification Level

Please attach a copy of NCCP certification to your application.  If you do not yet have your certification please advise when certification will be obtained.

þ Police Check

All team personnel are required to produce proof of a police check.  Coach applicants should contact Albert Rehanek, BMLA President (see contact info to right) for a Vulnerable Record Search Form accompanied by a signed letter from the BMLA. The Form must be completed and submitted to local police, along with the letter. If you have not received the stamped and returned police check at the time of application, a copy of the receipt issued by the police department should be included as proof that the Record Search is underway. The stamped and returned police check can then be given to the Program Coordinator when received. 

þ Policy Acknowledgement

Please download the “Policy Acknowledgement” document - date and sign, and attach to your coaching application.
