Presented annually since 1988 by the Ontario Lacrosse Referees Supervision Committee to the OLA Referee who combines on-floor officiating duties with program development efforts ar all levels

Presented annually since 1989 to the minor lacrosse referee selected for the best performance at a provincial championship

Awarded annually since 1960 for outstanding service to lacrosse

Presented annually since 1959 to the person selected as having done the most for the promotion of lacrosse in Ontario

Awarded since 1966 to the outstanding Midget goaltender
